What Is the N-648 Medical Waiver?

The N-648 is a medical form completed by a licensed medical professional to certify that an applicant has a physical, developmental, or mental impairment that prevents them from learning or demonstrating knowledge of English or U.S. civics.

 

This waiver is only granted when the condition:

  • Is medically determinable

  • Has lasted or is expected to last at least 12 months

  • Directly affects the ability to meet naturalization testing requirements

Who May Qualify for an N-648 Waiver

An applicant may qualify if they have a documented condition such as:

  • Cognitive impairment or intellectual disability

  • Dementia or memory-related disorders

  • Severe mental health conditions

  • Neurological conditions

  • Developmental disabilities

  • Other qualifying physical or psychological impairments

USCIS reviews each waiver individually. Approval is not guaranteed.

  • Any medical records related to your condition

  • Medication list (if applicable)

  • Immigration paperwork (if available)

  • Interpreter, if needed (unless provided by the clinic)
